Penile Discharge

Penile discharge refers to any unusual fluid that comes out of the opening at the tip of the penis (urethra). It is a common symptom that can occur in men of all ages and often indicates an infection or inflammation of the reproductive or urinary tract. While penile discharge can be concerning, most causes are treatable with appropriate medical care.

Symptoms of Penile Discharge

Symptoms associated with penile discharge may include:

  • Unusual fluid from the penis

    This may appear yellow, green, white, or clear and can vary in consistency from thin and watery to thick and pus-like.

  • Discomfort or pain

    You might experience a burning sensation when passing urine or a general discomfort in the genital area.

  • Frequent urination

    You may notice a need to urinate more often than usual, sometimes with urgency.

  • Itching or irritation

    The tip of the penis or the urethra may feel itchy or irritated.

  • Redness or swelling

    The penis, particularly around the opening of the urethra, may appear red or swollen.

  • Unusual odour

    The discharge might have an unpleasant or different smell than normal.

These symptoms often indicate infection or inflammation and should prompt a consultation with a healthcare provider.

Causes and Risk Factors

Penile discharge can result from various conditions, most commonly:

Sexually transmitted infections (STIs): Gonorrhoea and chlamydia are common infections that can cause discharge. These are typically transmitted through unprotected sexual contact with an infected partner.

Non-sexually transmitted infections: Urinary tract infections (UTIs) can sometimes cause discharge and are not necessarily related to sexual activity.

Balanitis: Inflammation of the head of the penis, often caused by poor hygiene, irritation from soaps, or underlying skin conditions.

Prostatitis: Inflammation of the prostate gland can sometimes lead to penile discharge, along with other urinary symptoms.

Multiple sexual partners: Having sexual contact with multiple partners increases the risk of exposure to infections that may cause penile discharge.

Unprotected sexual activity: Not using barrier methods of protection during sexual contact significantly increases the risk of infections.

Personal hygiene factors: Inadequate cleaning of the genital area or excessive use of irritating soaps and products can contribute to inflammation and discharge.

Types of Penile Discharge

Discharge can vary depending on the underlying cause:

Infectious discharge

Usually caused by sexually transmitted infections like gonorrhoea or chlamydia. This type often appears yellow or green and may be accompanied by pain during urination.

Inflammatory discharge

Results from inflammation rather than infection, such as from balanitis or reactions to irritants. This discharge is typically clear or whitish.

Non-infectious discharge

May be related to conditions like prostatitis or urethritis not caused by pathogens. This type varies in appearance but is often thin and clear or slightly cloudy.

Diagnostic Methods

If you experience penile discharge, a healthcare provider may use several diagnostic tools to identify the cause:

Medical history and examination: The doctor will ask about your symptoms, sexual history, and general health before examining the affected area to assess the discharge characteristics and any related symptoms.

Discharge sample analysis: A sample of the discharge may be collected and examined or sent to a laboratory for culture to identify any bacteria or other microorganisms.

Urine tests: You may be asked to provide a urine sample to check for infections or other abnormalities that could be causing the discharge.

STI screening: Specific tests for sexually transmitted infections such as gonorrhoea and chlamydia are often performed, which may involve swabbing the urethra or testing urine samples.

Treatment Options

Non-Surgical Treatment

For most cases of penile discharge, treatment focuses on addressing the underlying cause through non-invasive approaches:

  • Antibiotics: Prescribed for bacterial infections, including sexually transmitted infections like gonorrhoea and chlamydia. The type and duration of antibiotic treatment depend on the specific infection identified.
  • Anti-inflammatory medications: May be recommended to reduce inflammation and discomfort, particularly for conditions like balanitis or non-infectious urethritis.
  • Antifungal treatments: Used when fungal infections such as candidiasis are identified as the cause of the discharge. These may be applied directly to the affected area.
  • Prostate medications: For discharge related to prostatitis, medications that target prostate inflammation or infection may be prescribed, such as alpha-blockers or specific antibiotics.
  • Topical treatments: Creams or ointments may be recommended for external inflammation or irritation of the penis, particularly for conditions like balanitis.

Surgical Treatment

Surgical intervention is rarely needed but may be recommended if non-surgical measures fail:

  • Circumcision: Removal of the foreskin may be recommended in cases of recurrent balanitis or other persistent inflammatory conditions that do not respond to medical treatment.

Prevention and Management

Preventing penile discharge involves practising good personal hygiene, using barrier protection during sexual activity, having regular sexual health check-ups (especially with new partners), and seeking prompt medical attention if symptoms develop. For recurring conditions, following the treatment plan prescribed by healthcare professionals, completing the full course of any medications, and attending follow-up appointments helps manage the condition effectively.

Dr. Taufiq began his medical career as a Medical Officer under MOHH from 2006 to 2014, gaining valuable experience in multidisciplinary healthcare at Alexandra Hospital, KK Hospital, Bright Vision Hospital, and AMK THK Hospital.

From July 2017 to March 2021, he served as Chief Resident Physician at DTAP Clinic Somerset Branch, focusing on men’s health and providing holistic, patient-centered care.

As Medical Director at Pulse Medical Centre from April 2021 to December 2024, Dr. Taufiq led multidisciplinary care and advanced patient-first practices.

He holds certifications in ShangRing Circumcision, ZSR Stapler Circumcision, SCOPE certification, and ShangRing Training (Wuhu, Snnda, 2018). He is also a Certified Trainer for ShangRing Circumcision, ZSR Stapler Circumcision, Touchstone Circumcision, and the Storz T-Top Duolith Shockwave Therapy Machine.

In 2024, he completed the Androcourse under the Society of Men’s Health Singapore. Dr. Taufiq is a member of the Society of Men’s Health Singapore (SMHS), the Singapore Association for the Study of Obesity (SASO), and the International Society of Sexual Medicine (ISSM), reflecting his ongoing commitment to men’s health and weight management.
