Circumcision is a surgical procedure that removes the foreskin, which is the tissue covering the head of the penis. It is most commonly performed on newborn males, but it can also be performed on older children and adults for medical, cultural, or personal reasons.

What Is Male Circumcision?

Male circumcision is a minor surgical procedure that removes the prepuce, the retractable fold of skin covering the glans, or head, of the penis. It can be performed at any age, from newborns to adults, though the technique and considerations shift depending on how old the patient is. Once done, the glans is permanently exposed. It is a routine operation when carried out by qualified professionals, and it is fairly common here. A global prevalence study of male circumcision published in Population Health Metrics (Morris et al., 2016) estimated that approximately 14.9% of men in Singapore have undergone the procedure.

Indications for Circumcision

Circumcision may be recommended for various reasons. Common indications include:

  • Medical Reasons: Circumcision may be recommended for conditions such as:
    • Phimosis: Where the foreskin cannot be retracted and may cause discomfort or lead to infections
    • Recurrent Balanitis: Where the head of the penis and foreskin become repeatedly inflamed, causing soreness, swelling, and irritation that keeps returning.
  • Personal Preferences: Some individuals may opt for circumcision based on personal lifestyle choices or considerations related to hygiene and health.
  • Cultural and Religious Practices: Circumcision is often performed as part of cultural or religious customs in various communities.

Benefits of Circumcision in Singapore

Circumcision offers several potential health and hygiene benefits, which can be factors for individuals considering the procedure.

  • Reduced Risk of Infections

    Circumcision can decrease the chance of urinary tract infections during infancy and may lower the risk of acquiring certain sexually transmitted infections in adulthood.

    The procedure can prevent specific conditions such as phimosis, paraphimosis, and balanitis, which may otherwise lead to discomfort or the need for medical intervention.

When Circumcision Is Not Recommended

Certain physical conditions, such as hypospadias (where the opening of the urethra is not at the tip of the penis), may require the foreskin for future corrective procedures. In these cases, circumcision is typically postponed or avoided.

Prematurity or Health Instability

Circumcision may not be advisable for premature infants or babies with existing health issues until they are medically stable.

Circumcision Techniques

There is no single way to perform a circumcision. The right technique depends on the patient’s age, anatomy, and preferences, and each method has its own approach to comfort, healing, and results. Here are the main options:

Conventional Circumcision

The traditional surgical method, where the foreskin is removed with a scalpel and the wound is closed with dissolvable stitches. It typically suits patients of most ages and remains a dependable, well-understood approach.

Laser Circumcision

Laser circumcision uses a fine electrocautery tool rather than a conventional blade to remove the foreskin. It tends to involve less bleeding, as the tissue is sealed as it is cut, which can support a tidier recovery.

ZSR Stapler Circumcision

Stapler circumcision uses a single-use device that removes the foreskin and seals the edge with a ring of tiny staples in one motion. It is generally quick, involves minimal bleeding, and is popular with adults for its speed and neat outcome.

Plastibell Circumcision

With a Plastibell circumcision, a plastic ring is fitted over the glans and the foreskin is tied off, so the tissue gradually separates and the ring drops away on its own. This method of circumcision is most often used for infants.

Shang Ring Circumcision

This is a minimally invasive circumcision method that uses a specially designed ring to hold and remove the foreskin cleanly. It is generally quick, stitch-free, and known for a comfortable, straightforward recovery.

Surgical Circumcision Technique Used at TAF Clinic

The Circumcision Procedure

Pre-Operative Considerations

Before the procedure, the doctor will explain the process, including the benefits and potential risks, and obtain informed consent. A health assessment may be conducted, and anaesthesia options will be discussed based on the patient’s age, health status, and personal preferences. In many cases, male circumcision can be scheduled as a same-day procedure for convenience.

There are a few simple things you can do to prepare:

  • Wear loose, comfortable clothing on the day, and avoid tight underwear or trousers that could press on the area afterwards.
  • Have a light meal beforehand unless your doctor advises otherwise.
  • Arrange loose-fitting clothes and clean, supportive underwear to change into for the journey home.
  • Follow any specific instructions your doctor gives about medications or fasting.

Procedure Steps

The procedure starts with sterilising the surgical area to reduce infection risk, followed by administering anaesthesia to ensure comfort. Once numb, the chosen surgical technique is used to remove the foreskin. Bleeding is controlled throughout, and the wound is closed with sutures or supportive devices as needed to aid in healing.

Post-Operative Care

After the procedure, the patient is monitored for any immediate signs of complications, such as excessive bleeding or swelling. Proper wound care includes keeping the area clean, applying protective ointments, and using appropriate pain relief methods to manage discomfort.

Wound Care, Recovery, and Follow-Up

The recovery timeline for male circumcision varies: infants generally heal within 5 to 10 days, while older children and adults may take a few weeks. Minimally invasive techniques, such as the Shang Ring or the stapler method, may lead to quicker recovery. Follow-up visits may be recommended to ensure proper healing and address any questions or concerns.

A few things to keep in mind as you heal:

  • Hygiene and Care: Keep the area clean and dry, wash gently as directed, and apply any prescribed ointment to support healing.
  • Returning to Activity: Avoid heavy lifting, sport, and vigorous exercise for a few weeks, or until your doctor gives the go-ahead.
  • Erection Management: Nighttime erections are normal and can feel uncomfortable as the skin heals. Wearing loose underwear and following your doctor’s advice helps ease this.
  • Return to Sexual Activity: Adults are usually advised to wait until the area has fully healed, generally around 4 to 6 weeks, or as advised by your doctor.

Potential Risks and Complications

Circumcision in Singapore is generally considered a safe procedure, but there are some risks. These include common issues like minor bleeding, swelling, or infection, which are usually managed with standard medical care. Rare complications, such as injury to the glans or excessive removal of skin, are uncommon but may require prompt medical attention.

Dr Taufiq began his medical career as a Medical Officer under MOHH from 2006 to 2014, gaining valuable experience in multidisciplinary healthcare at Alexandra Hospital, KK Hospital, Bright Vision Hospital, and AMK THK Hospital.

From July 2017 to March 2021, he served as Chief Resident Physician at DTAP Clinic Somerset Branch, focusing on men’s health and providing holistic, patient-centred care.

As Medical Director at Pulse Medical Centre from April 2021 to December 2024, Dr Taufiq led multidisciplinary care and advanced patient-first practices.

He holds certifications in ShangRing Circumcision, ZSR Stapler Circumcision, SCOPE certification, and ShangRing Training (Wuhu, Snnda, 2018). He is also a Certified Trainer for ShangRing Circumcision, ZSR Stapler Circumcision, Touchstone Circumcision, and the Storz T-Top Duolith Shockwave Therapy Machine.

In 2024, he completed the Androcourse under the Society of Men’s Health Singapore.     Is circumcision painful?

    With appropriate anaesthesia, pain is minimised during the procedure, making it generally well-tolerated. For recovery, pain management options, such as oral pain relievers, are available to help manage any discomfort.

    Will urination be painful after circumcision?

    You may feel mild stinging or discomfort when urinating in the first few days, as the glans is more sensitive once the foreskin is removed. This usually eases quickly as healing progresses. Keeping the area clean and following your doctor’s aftercare advice helps keep any discomfort to a minimum.

    What age is best for circumcision?

    Circumcision is often simpler and heals faster in newborns, but the procedure can be performed at any age. For older children and adults, doctors may use specialised techniques to minimise discomfort and ensure proper healing.

    How soon can regular activities be resumed after circumcision?

    Children and adults should avoid strenuous physical activities for a few weeks. Adults are generally advised to wait until the surgical site has healed fully before resuming sexual activity, usually after 4 to 6 weeks or as recommended by the doctor.

    Can I use my Child Development Account (CDA) funds for circumcision at TAF Clinic?

    Yes, you can. TAF Clinic is a Baby Bonus approved institution, and CDA funds can be used for male circumcision procedures done at our clinic. (for AGES 6 & UP ONLY)

    Is circumcision covered by insurance in Singapore?

    It depends on your policy and the reason for the procedure. When circumcision is medically necessary, some insurance plans may help with the cost, though TAF Clinic is not MediSave or CHAS accredited. Procedures done for personal preference are often not covered. We recommend checking with your insurer, and our team can explain our pricing clearly.

    Will circumcision affect sexual function or pleasure?

    For most men, circumcision does not reduce sexual function or pleasure, and many notice no lasting difference once fully healed.
