Hepatitis B Testing and Vaccination

Hepatitis B is a liver infection caused by the Hepatitis B virus (HBV), affecting millions globally. The infection can be acute, lasting a few months, or develop into a chronic condition that increases the risk of cirrhosis and liver cancer. In Singapore, widely available screening and vaccination programmes help reduce the virus’ spread and prevent its complications.

What is Hepatitis B?

Hepatitis B is a viral infection that spreads through contact with infected blood or bodily fluids, such as during childbirth, unprotected sexual activity, or sharing needles. The virus can cause acute illness or progress into a chronic condition, increasing the risk of severe liver damage, including cirrhosis and liver cancer. Screening and vaccination are effective tools to prevent the spread of the virus and its long-term health effects.

Symptoms of Hepatitis B

Symptoms of Hepatitis B may vary, and some individuals may show no signs of illness. For those who experience symptoms, they typically appear 1 to 4 months after exposure. Common symptoms include:

  • Jaundice: Yellowing of the skin and eyes caused by liver dysfunction, often accompanied by dark urine and pale stools.
  • Fatigue: Persistent and unexplained tiredness that does not improve even with rest or reduced activity levels.
  • Abdominal Pain: Aches or discomfort, usually on the upper-right side of the abdomen near the liver area.
  • Dark Urine and Pale Stools: Changes in urine and stool colour, with urine appearing darker and stools lighter than usual.
  • Nausea and Vomiting: Feelings of queasiness that may lead to vomiting, often interfering with normal eating habits.
  • Loss of Appetite: A reduced desire to eat, which may contribute to decreased overall food intake and potential weight loss.

Who Should Be Tested?

Hepatitis B testing is recommended for individuals at higher risk, including healthcare workers frequently exposed to blood, those with multiple sexual partners, and people who inject drugs. Pregnant women are advised to test to reduce the risk of transmitting the virus to their newborns. Additionally, individuals born in or travelling to regions with high Hepatitis B prevalence should consider testing to manage potential risks.

Why Get Tested?

Testing for Hepatitis B allows for early detection, which is key to managing the infection and reducing health risks. Early identification helps healthcare providers monitor liver function, implement management strategies, and prevent serious complications like cirrhosis or cancer.

Hepatitis B Testing Options

There are different tests available to determine Hepatitis B status, immunity, and disease progression.

  • HBsAg Test

This test identifies the Hepatitis B surface antigen, which indicates an active infection.

  • Anti-HBs and Anti-HBc Tests

These antibody tests assess immunity or detect previous exposure to the virus.

  • Viral Load Test

Measures the amount of Hepatitis B virus in the blood, helping guide treatment for chronic cases.

  • Liver Function Tests

Additional blood tests evaluate liver health and detect any liver damage caused by the virus.

Vaccination Against Hepatitis B

Vaccination provides long-term protection against Hepatitis B by stimulating the immune system. In Singapore, it is part of the National Childhood Immunisation Schedule (NCIS) and National Adult Immunisation Schedule (NAIS).

  • Dosing Schedule for Children: The vaccine series starts at birth and is completed within the first six months.
  • Dosing Schedule for Adults: Adults typically receive three doses. The second dose is administered one month after the first, with the third dose given six months later.

Unvaccinated individuals, particularly those at higher risk, are strongly encouraged to receive the vaccine.

Complications of Hepatitis B

Chronic Hepatitis B, if untreated, can result in severe liver-related conditions. One such complication is liver cirrhosis, where permanent scarring impairs the liver’s ability to function effectively. Over time, the damage may progress to liver failure, where the liver can no longer perform its necessary functions. Additionally, chronic infection significantly increases the risk of developing liver cancer, highlighting the long-term impact of the disease on overall health.

    Frequently Asked Questions

Can I get Hepatitis B from casual contact?

No, Hepatitis B is not spread through casual interactions such as hugging, shaking hands, or sharing food and utensils. The virus requires direct contact with infected blood or bodily fluids to be transmitted, so everyday social contact poses no risk of infection.

Can I get vaccinated if I am already infected?

Vaccination is not effective for individuals with an active Hepatitis B infection, as it is designed to prevent the virus rather than treat it. However, for those who are not infected or have not been exposed to the virus, vaccination is strongly recommended to provide long-term protection against Hepatitis B.

Can I get Hepatitis B more than once?

Once a person recovers from an acute Hepatitis B infection, they develop lifelong immunity to the virus and cannot get it again. However, individuals who are chronic carriers of Hepatitis B remain at risk for complications such as liver damage and should seek ongoing medical care to monitor their condition.
