What is Doxy-PEP?

Doxy-PEP combines two ideas. The first is a prescription antibiotic, doxycycline, used in this specific context off-label. The second is post-exposure prophylaxis (PEP), a medical term for taking a medication after a possible exposure to lower the chance of an infection developing.

Doxycycline itself is a tetracycline-class antibiotic that has been used for decades. It is well-established in the treatment of conditions such as acne, respiratory infections, and Lyme disease. In Singapore, doxycycline is prescribed on an off-label basis, which means it is being used for a purpose beyond its original approved indications. Off-label prescribing is common, legal, and evidence-based in medicine when supported by clinical research, as is the case here.

The antibiotic targets bacterial STIs specifically. It has been shown to reduce the risk of chlamydia and syphilis significantly, and it offers some protection against gonorrhoea, though this effect is less consistent because of rising antibiotic resistance in the bacteria that cause gonorrhoea. Doxy-PEP does not protect against viral STIs such as HIV, herpes, or HPV, and it does not act as a treatment for infections that have already been established.

Because doxycycline is a prescription-only medication in Singapore, Doxy-PEP is not something you can obtain independently. A consultation is required, both to confirm eligibility and to make sure the medication is being used safely each time.

How Effective is Doxy-PEP?

Doxy PEP offers an effective, evidence based way to reduce the risk of bacterial STDs like chlamydia, syphilis, and gonorrhoea, particularly for individuals in higher risk groups. When taken correctly within 72 hours after potential exposure, it has been shown to lower the risk of infection meaningfully, with the risk of Syphilis being reduced by around 70 to 80 percent, and Chlamydia risk by around 70 to 90 percent. The picture for gonorrhoea is more variable. Some trials have shown a reduction of around 50 to 55 percent, while others have shown little to no effect. This variation is largely explained by differing levels of tetracycline resistance in gonococcal isolates across different populations, which is one reason gonorrhoea prevention should not be assumed on Doxy-PEP alone.

How to Take Doxy-PEP

Doxy-PEP is taken as a single 200mg dose of doxycycline in pill form after condomless sex. In Singapore, this is prescribed by a doctor rather than kept as a personal stock, though patients with recurring risk factors may discuss an advance prescription with their doctor during a follow-up.

The dose should be taken as soon as reasonably possible after the exposure, ideally within 24 hours and no later than 72 hours after. Beyond that window, the medication is unlikely to be effective, since bacterial infection may already have taken hold.

Doxy-PEP is event-based rather than daily. It is used only after a specific potential exposure, which is an important distinction from ongoing preventive medications you may have come across.

To help the pills work as intended and minimise stomach discomfort:

  • Take the dose with a full glass of water.
  • Stay upright, sitting or standing, for 30 to 60 minutes after swallowing the pills, to reduce the risk of oesophageal irritation.
  • Separate the dose from dairy products, calcium supplements, iron supplements, and antacids by at least two hours, as these can interfere with absorption.
  • Avoid prolonged sun exposure or use sunscreen where possible, since doxycycline can increase skin sensitivity to sunlight.

Doxy-PEP vs PrEP: What’s the Difference?

Doxy-PEP and PrEP sound similar and are often mentioned in the same conversation, but they are different medications used for different purposes.

Doxy-PEP

Doxy-PEP is an antibiotic (doxycycline) taken after possible exposure to lower the risk of bacterial STIs such as chlamydia and syphilis. It is used on an event basis, meaning it is taken only after a specific incident of condomless sex.

PrEP

PrEP, or pre-exposure prophylaxis, is an antiretroviral medication taken before potential exposure to prevent HIV infection. It is used on an ongoing basis, either daily or as an on-demand regimen, depending on what is agreed with your doctor.

The two are not interchangeable. Doxy-PEP does not prevent HIV, and PrEP does not prevent bacterial STIs.
